Red cell distribution width (RDW) could be of interest by its potential use in the assessment of celiac disorder (CD). The main objective of this study was to evaluate the case positive rate of CD and the utility of red cell distribution width (RDW) in the CD diagnosis. This prospective study included 9.066 middle adult (≥45 years old) and elderly patients (≥60 years old) from 2012 to 2021. CD diagnosis was performed by CD antibody tests (serology and Human Leucocyte Antigen genotype (HLA)) and biopsy. Gastrointestinal and extra-intestinal manifestations as well as hematological and biochemical parameters were analyzed. CD diagnoses were confirmed in 101 patients (median (IQR) age = 62 (52.3−73); 68.32% women) by serologic tests (100%) and intestinal biopsy (88.12%), showing mainly marked or complete atrophy (76.24%, MARSH 3a−c). Anemia was the most commonly presenting extra-intestinal manifestation (28.57%). Among 8975 individuals without CD, 168 age and sex matched were included. By comparison of CD and no CD individuals, we observed that high >14.3% RDW was exhibited by 58.40% and 35.2% individuals with CD and without CD, respectively. Furthermore, high RDW is associated with CD and grade III atrophy. We suggest that RDW could be used as a CD screening criterion.

BACKGROUND: External Quality Assessment (EQA) programs on urinary sediment (U-sed) examination are recommended by international guidelines to improve the quality of this diagnostic tool. However, no such programs have been carried out so far in developing countries of sub-Saharan Africa. This paper describes an EQA program on U-sed which was realized in 2017 and 2018 by means of innovative communication technologies in Benin Republic, a small country of West Africa. METHODS: On Monday morning, weekly in 2017 and fortnightly in 2018, the image of one U-sed element was sent from Italy with a smartphone, via WhatsApp application, to a group of 13 professionals (11 bachelors of science, 2 medical doctors) working in different institutions of Benin Republic. The image showed one U-sed element, for which the participants were asked the identification and, in 2018, also a clinical association. RESULTS: The images of 33 elements were submitted over the two-year period. Particle identification. Altogether 283 answers were received (8.5 ± 2.3/single image), 200 of which were correct (70.7%), 53 incorrect (18.7%), 17 partially correct (6.0%), and 13 "I don't know" (4.6%). Correct identification of elements presented twice (2nd answer vs. 1st). No change: 9/18 (50.0%); improvement: 5/18 (27.8%); worsening: 4/18 (22.2%). Clinical association. Correct answers: 8/17 (47%); incorrect: 8/17 (47.0%); no answer: 1/17 (5.6%). CONCLUSIONS: This paper demonstrates: 1. The need and utility of an EQA program on U-sed in developing countries of sub-Saharan Africa; 2. The feasibility of such programs by means of easily available and inexpensive smartphone applications.

Acanthocytes, when≥5% of urinary erythrocytes examined, are considered as the most reliable marker of glomerular haematuria. However, some uncertainties still exist in the literature about their morphological definition. The aim of this paper is to discuss this topic and to suggest a univocal definition of acanthocytes as erythrocytes with the shape of a ring with one or more protrusions

Sin embargo, todavía existen algunas incertidumbres en la literatura acerca de su definición morfológica. El objetivo de este artículo es discutir este tema y proponer una definición unívoca de los acantocitos como eritrocitos con la forma de un anillo con una o más protuberancias

The authors studied the inaccuracy effect in the determination of C(min) and C(1h) post-infusion serum concentrations of vancomycin, amikacin, and tobramycin on the recommended dose regimen (RDR) using the Abbottbase Pharmacokinetic Systems (PKS) program (Abbott; Abbott Park, IL). According to previously established criteria, the clinically acceptable error (CAE) was defined as 1/8 of the therapeutic range. For a total of 647 simulations, in most cases (94.3%) an inaccuracy of up to three times the CAE did not lead to changes in the RDR. However, and particularly for amikacin and tobramycin, in some cases an inaccuracy in the order of the CAE in C(min) lead to important differences in the RDR, which could have important consequences in clinical practice. For therapeutic monitoring of these antibiotics, it is suggested that a serum concentration from a previous moment in time, which may be determined with greater precision and accuracy, could be used instead of C(min).
